What to expect under ‘My Home Health’ Care ?

Home health care is healthcare services that are delivered in a patient’s home, which is  recommended by a doctor for one of the following reasons:

  • Where an individual was recently discharged from a hospital or long-term care facility, such as a nursing home or rehabilitation center
  • Where a doctor determines that an individual has a need for nursing or therapy at home
  • When there is a very little or no possibility for further improvement at hospital, so you want to bring back the patient to home with possible support.
  • When the cost of hospital treatment and possible outcome is not convincing enough to extend the hospital stay.

Home health care helps patients get better, reach a level of independence and become as self-sufficient as possible. For those with long-term health problems, the goal is to maintain the highest level of ability and learn to live with an illness or disability.
